Position Overview:

The Advanced Practice Nurse will collaborate closely with the supervising physician to deliver thorough health evaluations and formulate individualized care plans for our diverse patient demographic.

Responsibilities include maintaining accurate medical records by documenting subjective and objective findings, assessments, and treatment plans in a clear, concise, and timely manner.

Additionally, the role involves providing education to patients and their families, as well as exercising prescriptive authority in partnership with the supervising physician.

Essential Qualifications:
Current New York State licensure as a Nurse Practitioner or Physician Assistant is mandatory (the NP specialty certification must align with the area of practice).

Candidates must possess an NPI number and be enrolled in Medicare with a Medicare Identification Number (or PTAN) OR be eligible for Medicare Enrollment within six months of employment (eligibility for NP requires National Certification through AANP or ANCC, and a Master's Degree; eligibility for PA requires graduation from an accredited PA program and National Certification through NCCPA).

Initial certification must be achieved or obtained within six months of employment. A valid CPR certification from the American Heart Association or the American Red Cross is also required.

Salary Range:
$92,000 - $147,004

Please note that the salary information provided is a general guideline. Salaries are determined based on the candidate's skills, experience, and qualifications, as well as internal equity, market, and business considerations.

The salary range indicated is based on full-time employment, 1.0 FTE, and would be prorated for less than 1.0 FTE.
