Job Summary

We are seeking a highly skilled Care Coordinator to join our team at Memorial Healthcare System. As a key member of our healthcare team, you will be responsible for coordinating the transfer and bed placement of patients across our facilities.

Key Responsibilities:

  • Coordinate patient transfers and bed placements to ensure efficient and effective care delivery
  • Communicate effectively with healthcare teams, including surgery, emergency department, nursing units, and physician offices
  • Monitor and document transfer needs, and consult with other disciplines as needed
  • Ensure compliance with EMTALA and other regulatory requirements
  • Collaborate with healthcare teams to coordinate patient care and optimize outcomes

Requirements:

  • Bachelor's degree in Nursing (or related field)
  • Minimum 3 years of experience in Emergency Department or Critical Care
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
