Role Overview

Upperline Health is looking for a dedicated Nurse Practitioner to deliver care in both clinic and home settings.

Working closely with our local physician team, you will create comprehensive care plans that align with patients' health goals and values, while providing diagnostics and treatments in various settings, including telehealth.

Your role will also involve educating patients and their families on medication management, nutrition, and preventive care strategies.

Key Responsibilities:

  • Formulate and execute clinical care plans for adults with chronic health issues (e.g., diabetes, COPD, CHF, dementia)
  • Conduct thorough patient assessments, including history and physical examinations
  • Collaborate with the interdisciplinary team to address both physical and psychosocial needs of patients and their families
  • Provide compassionate care tailored to complex patient situations
  • Educate patients and families on medication, dietary needs, and crisis prevention
  • Maintain clear communication with other healthcare providers to ensure coordinated care
  • Document patient interactions and care plans in compliance with regulatory standards
  • Participate in team meetings and educational sessions
  • Prescribe medications as permitted by state regulations
  • Engage in ongoing professional development and education
Qualifications:

  • Master's degree in nursing is essential
  • Board Certification as an Adult, Adult-Geriatric, or Family Nurse Practitioner is required
  • Active Florida State Registered Nurse & Nurse Practitioner license is mandatory
  • Minimum of 2 years of experience in primary care or internal medicine
  • Experience in value-based care is advantageous
  • DEA registration or eligibility is necessary
  • Basic Life Support (BLS) certification is required
  • Must be comfortable providing care in patients' homes
  • Strong communication skills are vital for effective patient interactions
