Key Responsibilities- Review and verify the accuracy of medication orders and medication history, making any necessary adjustments.
- Perform accurate clinical assessments of medication orders.
- Ensure the safety and responsible use of medications through medication reviews and monitoring, providing education and consultation.
- Collaborate with other medical professionals to optimize therapy and provide quality medical care.
- Provide patient and provider education on medication use, side effects, drug therapy, and indications of medications.
- Work with the hospital's pharmacological, nursing, and medical staff to ensure patients receive the best possible medication use.
- Graduate from an accredited pharmacy college.
- Licensed as a Clinical Pharmacist in the state of California.
- Hold a Healthcare Providers Certification (HCP) Certificate.
- Minimum of 2 years experience working as a clinical pharmacy practitioner.
- Excellent communication and interpersonal skills.
- Knowledge of current pharmacy practice and standards.
