Position Overview:

The Nurse Practitioner will be duly licensed in the State of Mississippi and is expected to operate under a protocol sanctioned by a licensed physician in Mississippi, in collaboration with the Board of Trustees. This role functions under the guidance of the Executive Director of Post Acute Care and the Chief Medical Officer, working closely with the Medical Director of Palliative Care.

Key Responsibilities:

  • Deliver comprehensive palliative care aimed at symptom alleviation while engaging in discussions about care objectives to ensure patients and families comprehend their medical conditions, treatment possibilities, and care goals.
  • Consult with the Medical Director of Palliative Care regarding the management of assigned patients.
  • Keep all medical personnel and caregivers updated on patient care needs and issues.
  • Collaborate with discharge planning teams to improve the continuity of palliative care following discharge.
  • Engage in weekly Interdisciplinary Team Meetings, Palliative Care Committee meetings, and other relevant gatherings.
  • Work cooperatively with other staff to fulfill the aims of palliative care and maintain positive relationships across departments and with the public.
  • Act as a unified representative of the palliative care program when interacting with medical and hospital staff.
  • Participate in or lead educational initiatives for hospital personnel regarding palliative care.
  • Safeguard patient rights and maintain confidentiality of patient information, adhering to the We CARE values.
  • Exhibit the knowledge and skills necessary to effectively communicate and engage with patients, families, and visitors of diverse age groups, being mindful of their cultural and religious values.
  • Perform additional duties as necessary.

Performance Expectations:

Performance expectations will be outlined during the orientation and preceptor phases of your onboarding process.

  • Demonstrate consistent knowledge and proficiency in delivering palliative care to patients.
  • Exhibit the ability to collaborate effectively with interdisciplinary team members in patient care.
  • Show capability in working with interdisciplinary teams and external organizations to minimize care gaps.
  • Provide education and training to hospital staff on palliative care principles.
  • Manage multiple tasks efficiently and prioritize responsibilities effectively.
  • Display leadership qualities including vision, self-motivation, performance, determination, communication skills, credibility, ethical conduct, and the ability to inspire and achieve desired outcomes through teamwork.

Qualifications:

Education/Skills:

Must be a graduate of an accredited non-online RN program. A Master's Degree in Nursing is required.

Work Experience:

Prior experience as a Nurse Practitioner is preferred, with a minimum of 720 clinical hours required. Experience in Palliative Care, Gerontology, Acute Care, or Oncology is advantageous.

Certification/Licensure:

  • Must hold a valid APRN license from the Mississippi State Board of Nursing.
  • Certification as a Family Nurse Practitioner from the American Nurse Credentialing Center (ANCC) is required.

Additional Certification/Licensure:

  • Certification in Palliative Care - Advanced Certified Hospice and Palliative Nurse (ACHPN) is required within the first year of employment.

Mental Demands:

Exceptional interpersonal skills are essential. The ability to function as a team member, cooperate with colleagues, follow directions, demonstrate initiative, and perform under pressure is crucial.
