Key Responsibilities
  • Provide Skilled Nursing Services
    • Deliver professional nursing services under the direction of a RN in compliance with state and federal regulations, applicable licensure/certification requirements, and CenterWell Home Health policies and procedures.
    • Implement individualized care plans to address the unique needs of each patient, focusing on rehabilitation, recovery, and prevention.
  • Collaborate with Healthcare Team Members
    • Work closely with RNs, therapists, and other healthcare professionals to ensure seamless care coordination and effective communication.
    • Participate in care planning and goal-setting to achieve optimal patient outcomes.
  • Document Patient Care
    • Accurately and thoroughly document patient interactions, care provided, and outcomes in accordance with CenterWell Home Health policies and procedures.
    • Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements.
  • Meet Quality and Safety Standards
    • Adhere to CenterWell Home Health's quality and safety standards, ensuring a safe and healthy environment for patients and staff.
    • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements
  • Education and Licensure
    • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
    • Current nursing license in the practicing state.
  • Experience and Skills
    • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
    • Valid drivers license, auto insurance, and reliable transportation.
    • Current CPR certification.
