Care Coordination Associate

About the Role

The Care Coordination Associate at Cope Health Solutions provides telephonic and field-based case management services to clients enrolled in our Enhanced Care Management and Community Support Program. This role is responsible for identifying critical resources for clients, helping them navigate through healthcare services and systems, and promoting client health.

Key Responsibilities
  • Establish close relationships with and serve as a point of contact for clients.
  • Deliver weekly or monthly health education and promote self-management to clients.
  • Communicate with care team members to facilitate client care.
  • Identify resources for clients to overcome barriers to care.
  • Maintain strict confidentiality in accordance with agency policies.

$22.00 - $25.00 per hour salary reflects the competitive compensation package offered by Cope Health Solutions.
