Job Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team of healthcare professionals. The successful candidate will be responsible for delivering personalized care to patients, collaborating with physicians, and working closely with family members to ensure optimal patient outcomes.

Key Responsibilities
  • Develop and implement individualized care plans tailored to each patient's unique needs and goals.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Conduct thorough assessments and coordinate care with other healthcare professionals.
  • Provide health education and training to patients and their families.
  • Maintain accurate records of patient care and communicate effectively with Case Managers.
Requirements
  • Current LPN licensure required.
  • At least 6 months of prior hands-on nursing experience preferred.
  • Ability to work independently and collaboratively as part of a multidisciplinary team.
