Medical Professional

The Family Medicine LPN is a critical member of our healthcare team, responsible for assessing, planning, and providing high-quality nursing care to patients in an outpatient clinical environment. With a strong focus on critical thinking and the nursing process, this role requires a skilled and compassionate professional who can work effectively in a fast-paced setting.
Key Responsibilities
* Assess patient needs and develop individualized care plans
* Provide direct patient care, including medication administration and wound care
* Collaborate with healthcare teams to ensure seamless care transitions
* Maintain accurate and detailed patient records
Requirements
* 2+ years of current Family Medicine/Clinic experience
* Clinical Office Experience
* PHONE Triage experience
* EPIC EMR experience (highly preferred)
* Previous Travel experience (preferred)
