Job Summary

We are seeking a Registered Nurse to join our interdisciplinary team as a Hospice Care Manager. As a key member of our team, you will provide high-quality patient care and services, visiting patients in their homes, skilled nursing facilities, and assisted living facilities.

Responsibilities

  • Provide comprehensive patient assessments, interventions, and care planning in collaboration with the interdisciplinary team.
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
  • Collaborate with healthcare professionals, social workers, chaplains, and volunteers to ensure seamless patient care.
  • Manage patient records, documentation, and communication with the healthcare team.
  • Participate in ongoing education and training to stay current with hospice regulations, best practices, and industry developments.

Requirements

  • Current Registered Nurse licensure (in good standing) in the state of Ohio.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
  • Strong time management and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities.
  • Ability to learn and utilize electronic medical records systems.
  • Reliable transportation, a valid driver's license, and auto insurance.
