We are seeking a highly skilled and compassionate Care Manager to join our team at RHA Health Services. As a Care Manager, you will play a vital role in providing whole-person care to individuals with intellectual and developmental disabilities, mental health, and substance use needs.
Key Responsibilities- Conduct comprehensive assessments and develop tailored care plans to meet the unique needs of each individual.
- Collaborate with integrated care teams, community organizations, and RHA departments to ensure seamless care coordination.
- Implement best practices and stay up-to-date with current research and innovations in care management.
- Ensure high-quality care delivery and services, adhering to company, state, and federal policies.
- Communicate effectively with individuals, families, and team members to promote person-centered care and address concerns.
- Participate in quality improvement initiatives and contribute to a culture of continuous learning and improvement.
- Master's degree in a human service field or related field, or equivalent experience.
- At least one year of full-time experience working with the MH/SU/IDD population, or equivalent experience.
- Valid driver's license and reliable transportation.
- Clinical license preferred.
- A competitive salary of $25.00 per hour.
