Job Summary

We are seeking a highly skilled Clinical Specialist to join our Assertive Community Treatment (PACT) team at the Cumberland County Guidance Center. As a key member of our team, you will play a vital role in providing evidence-based case management and advocacy services to our clients.

Key Responsibilities

  • Develop and implement individualized recovery plans for clients
  • Collaborate with PACT team staff and other providers to ensure comprehensive care
  • Provide evidence-based individual and group counseling to support client goals
  • Conduct emergency mental health assessments and provide crisis stabilization services as needed
  • Consult with medical staff regarding client medication concerns and issues
  • Provide education, consultation, and support services to family members and support persons of PACT clients
  • Participate in quality improvement processes to enhance PACT team services
  • Assess client needs, services delivery, and evaluate client progress towards goals
  • Provide education to clients and employees
  • Collaborate with clients' support networks and community providers
  • Provide transportation and services to increase travel independence
  • Participate in on-call rotation with other PACT employees
Requirements

To be successful in this role, you will need:

  • A Master's degree from an accredited college/university in a human service discipline (social work, psychology, or related human services field)
  • A minimum of 2 years' post-bachelor's experience in the provision of mental health services and working with consumers with severe and persistent mental illness
  • Knowledge of community resources and agencies
  • A valid automobile driver's license with a MVR that meets agency standards
  • Willingness to work a 40-hour flexible weekly schedule
  • Spanish speaking preferred
