Key Responsibilities
  • Implement and coordinate patient care plans in accordance with physician orders and individual patient needs.
  • Conduct thorough patient assessments and develop effective care plans to achieve optimal patient outcomes.
  • Administer medications, treatments, and therapies as prescribed by physicians, and document all care provided in a timely and accurate manner.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
