Evidence-Based Medical Care
As a Clinical Care Specialist, you will provide evidence-based medical care to seniors and patients with complex needs, assessing and treating various medical conditions while collaborating with other healthcare professionals to create comprehensive care plans. You will be responsible for examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses. Your duties will include ordering and interpreting laboratory and diagnostic testing as needed, providing referrals to other healthcare practitioners when necessary, and ensuring patient medical records are kept up to date.

Requirements and Qualifications
An active state Nurse Practitioner license, current national board certification, and active DEA licensure are required. A Master's degree in Nursing (MSN) and five or more years of nursing experience, with at least two years of nurse practitioner experience, are also preferred. Strong interpersonal communication skills, excellent time management skills, proficiency in medical instruments and equipment, and knowledge of HIPAA and OSHA regulations are essential. Proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications is also required. Additionally, a licensed driver with valid automobile insurance and the ability to engage in moderate physical activity are necessary.
