Job Summary

We are seeking a highly skilled Nurse Practitioner to join our team at AndHealth. As a key member of our healthcare team, you will play a vital role in helping patients reverse chronic disease through personalized treatment plans and collaborative care.

Key Responsibilities:

  • Develop and implement comprehensive treatment plans for patients, working closely with our health coaches and other care team members.
  • Utilize our proprietary clinical treatment protocol to ensure patients receive the best possible care.
  • Review and provide feedback on health coach-created treatment plans to ensure high-quality patient care.
  • Assess medication readiness and manage medications as needed for patients.
  • Order and interpret laboratory testing as required.
  • Monitor patient progress, engage with coaches to coordinate care, and serve as a clinical escalation point for coaches and other care team members.
  • Collaborate with other providers to ensure patients have access to the care necessary to reverse their chronic disease.

Requirements:

  • Nurse Practitioner degree required.
  • Specialization in Dermatology or Rheumatology preferred.

Preferred Qualifications:

  • Experience in family medicine, neurology, internal medicine, integrative medicine, or primary care.
  • Excellent written and verbal communication skills, with comfort managing patient relationships remotely using telehealth technology.
  • Ability to document patient information accurately and provide detailed clinical outcomes.
  • History of delivering patient outcomes through various treatment modalities.
  • Belief in the potential for reversing chronic diseases through root cause approaches.
  • Excitement about working within an innovative care model and leveraging technology to improve patient care.

Additional Requirements:

  • Proof of negative TB skin test within the last 12 months.
  • Measles, Mumps, and Rubella or A Blood Titer proving immunity.
  • Varicella - (2) documented doses or A Blood Titer proving.
  • Hep B3 Series.
  • Influenza required.
  • COVID Vaccine required.
  • Additional health requirements may also be required.
