Job Responsibilities- Conduct comprehensive in-home assessments to identify potential health risks and develop personalized care plans.
- Collaborate with our multidisciplinary team to ensure seamless care coordination and effective disease management.
- Perform physical examinations, medication reviews, and vital checks to ensure accurate diagnosis and treatment.
- Communicate assessment findings to our primary care team to inform care decisions and improve patient outcomes.
- Meet or exceed quality metrics and closure rates, ensuring high patient satisfaction and improved health status.
- Travel frequently to provide care to patients in their homes, as well as in clinic settings in residential care facilities.
We are seeking a qualified Nurse Practitioner or Physician Assistant with the following qualifications:
- FNP/Family Nurse Practitioner, APRN, Nurse Practitioner or Physician Assistant/PA degree.
- Unrestricted NP or PA license in the state of IN.
- Nurse Practitioner certification from either the ANCC / AANP.
- Current DEA licensure / Prescriptive Authority post-hire.
- Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS).
With a minimum of 2+ years of clinical experience as an NP/PA in primary care or home care settings, you will possess a working knowledge of chronic care management and various medication options. Experience in Family Medicine, Internal Medicine, Gerontology, and Palliative Care is highly desirable.
