Job Summary

We are seeking a skilled Care Manager to join our team at Loma Linda University Medical Center. As a key member of our patient care delivery team, you will play a vital role in ensuring the highest quality of care for our patients.

Responsibilities
  • Utilize clinical expertise to perform the nursing process, including assessment, care planning, intervention, and evaluation, within a patient-centered care professional practice model.
  • Maintain a solid working knowledge of specialized case and utilization management methodologies and practices, applying concepts to everyday practice.
  • Ensure medical appropriateness criteria are met for level of care provided and documented from admission through discharge.
  • Practice in a collaborative healthcare environment to oversee implementation of a well-thought-out interdisciplinary plan of care with an individualized discharge plan that meets the continuing healthcare needs of the patient.
  • Reevaluate plan of care and ensure continued appropriateness based on the patient's changing needs and condition.
Requirements
  • Bachelor's Degree in nursing required.
  • Minimum of one year of patient care experience required.
  • Minimum of two years of Hospital Case Management or Utilization Management experience preferred.
Preferred Qualifications
  • Ability to speak, read, and write legibly in English (and Spanish preferred) with professional quality.
  • Ability to use computer, printer, and software programs necessary to the position, e.g., Microsoft Office Suite, Outlook, electronic medical record, electronic event reporting program.
