Job Summary

We are seeking a skilled Case Manager to join our team at Loma Linda University Health. As a key member of our patient care delivery team, you will play a vital role in ensuring the best possible outcomes for our patients.

Key Responsibilities
  • Utilize clinical expertise to assess patient needs and develop individualized care plans
  • Collaborate with interdisciplinary teams to ensure seamless care transitions and optimal patient outcomes
  • Apply knowledge of case and utilization management methodologies to ensure medical appropriateness and efficient resource allocation
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care
  • Stay up-to-date with industry developments and best practices to continuously improve patient care and outcomes
Requirements
  • Bachelor's Degree in Nursing required
  • Minimum of one year of patient care experience required
  • Minimum of two years of Hospital Case Management or Utilization Management experience preferred
  • Active California Registered Nurse (RN) license required
  • Basic Life Support (BLS) certification issued by the American Heart Association required
