We are seeking a dedicated and compassionate Nurse to join our team at Ambercare, one of the largest Home Health Care companies in the State of New Mexico. As a Nurse with Ambercare, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the RoleAs a Nurse with Ambercare, you will be responsible for providing high-quality, patient-centered care to our clients in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide skilled nursing care to patients in their homes, including assessments, wound care, medication management, and education.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Provide education and support to patients and their families to promote health and wellness.
- Document patient progress and outcomes in a timely and accurate manner.
- Current licensure as a Nurse in the State of New Mexico.
- At least one year of experience as a Nurse in a home health care setting.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic computer skills and ability to navigate electronic medical records.
