Job Description

Clinical Admissions Specialist (RN)

As a Clinical Admissions Specialist (RN), you will play a crucial role in assessing patients in their place of residency or in hospitals, initiating care into our services, and ensuring a seamless transition to Care Navigation.

You will serve as an ambassador for our organization, communicating effectively with patients, families, physicians, and facilities to provide compassionate and empathetic support.

Responsibilities:

  • Completes clinical admissions for all Company services
  • Explains services to potential patients/families and addresses questions regarding patient fears and physical limitations
  • Identifies admission objections, concerns, or entry barriers and provides responses
  • Educates patient/family on admission requirements and addresses immediate needs
  • Provides information to Physicians and initiates Plan of Care to address patient's immediate needs
  • Initiates skilled nursing interventions to prevent complications, alleviate symptoms, and maximize physical and emotional comfort
  • Obtains Physician orders
  • Completes documentation per Company policy

Requirements:

  • Bachelor's degree in nursing or related field or equivalent combination of education and experience
  • Current Florida license as RN, current active single-state out-of-state nurse license, or current active multistate/compact nurse license
  • Minimum of two (2) years of RN experience in acute care, hospice, home health care, or other post-acute care
  • Strong clinical assessment skills with preferred background in emergency department or ICU
  • Previous experience working with an EMR/EHR system
  • Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred
  • Ability to present services in a professional and articulate manner

Salary: $65,000 - $80,000 per year, depending on experience
