">We are seeking a dedicated Home Health Physical Therapist to join our team at Interim HealthCare. As a Home Health Physical Therapist, you will have the opportunity to provide personalized care to patients in the comfort of their own homes.

">The salary for this position is estimated to be between $97k and $120k per year, based on full-time employment and experience.
">Job Description
">Your primary responsibility as a Home Health Physical Therapist will be to provide home-based physical therapy services to patients who require assistance with mobility, strength, and functionality due to illness or injury. You will work closely with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement individualized care plans that meet the unique needs of each patient.
">You will conduct thorough assessments, observe movements, document progress, and modify care plans as needed to ensure optimal outcomes. Your expertise will also include assisting patients with exercises and hands-on therapy to manage pain and increase mobility, educating patients on their plan of care, established goals, exercises, and self-care, and coaching family members on safely assisting with exercises.
">Required Skills and Qualifications
">To be successful in this role, you must possess a graduate degree from an accredited Physical Therapist program and be licensed to practice as a Physical Therapist in Colorado. A minimum of one year of physical therapy experience, preferably in home healthcare, is also required. Additionally, you must hold BLS/CPR certification and have knowledge of state and federal home health regulations.
