The Clinical Supervisor for Care Management plays a pivotal role in directing the daily operations of the Care Management Department. This position is responsible for offering guidance and support to the Care Management team, ensuring a balance between clinical, strategic, regulatory, risk management, and financial considerations in care coordination and discharge planning.

Key Responsibilities:

  • Oversee and assess the daily functions of the Care Management Department.
  • Manage Care Managers' assignments to guarantee appropriate staffing levels across patient care units, including processing time-off requests and scheduling.
  • Deliver ongoing, constructive feedback to Care Managers regarding their performance.
  • Recognize and celebrate the accomplishments of team members consistently.
  • Utilize conflict resolution and negotiation skills to address and resolve issues promptly.
  • Refer complex cases and concerns to the Director or Assistant Director of Care Management, adhering to established procedures.
  • Identify, plan, and coordinate care aspects for patients and their families.
  • Initiate necessary documentation to comply with regulatory standards.
  • Ensure timely and accurate documentation of all activities.

Position Requirements:

  • Graduate of an accredited Nursing School or College; a current New York State R.N. License is required, with a BSN or a Master's Degree in a related field preferred.
  • Experience in acute care Care Management is essential.
  • National Certification is preferred.
  • Management experience in Care Management is advantageous.
  • Basic proficiency in personal computers and software for word processing and spreadsheets is preferred, along with the ability to enter and retrieve data from relevant systems.
