We are seeking a skilled Licensed Practical Nurse to join our healthcare team at St. David's Healthcare at Home. Our nurses play a vital part in delivering high-quality, skilled nursing care to patients in their homes.

Key Responsibilities

The Licensed Practical Nurse will work in accordance with physician orders and the Plan of Care under the direction and supervision of a Registered Nurse and/or Director, Professional Services. Key duties include:

  • Providing patient care and performing ongoing assessments
  • Documenting data and assessing patient/caregiver's knowledge of disease process
  • Collaborating with the healthcare team to develop and implement effective care plans

Requirements

Candidates must have graduated from an accredited LPN/LVN program, have a minimum of one year experience in a clinical care setting, and be licensed as an LPN/LVN in good standing.
