Job Summary:
LifeMoves is seeking a highly motivated Case Manager to join our team at Georgia Travis House, a transitional housing site in Santa Clara County. As a Case Manager, you will act as a strong support and advocate for clients, helping them attain stable housing and self-sufficiency.

Key Responsibilities:
  • Develop and implement individual case plans for a caseload of clients, ensuring progress toward client goals.
  • Provide on-going supportive and/or case management functions in accordance with the case/treatment plan.
  • Communicate effectively with probation officers, social workers, and family members as needed.
  • Participate actively as a team member in supporting clients and the facility as needed.
  • Stay current and knowledgeable about external community resources, referrals, and service linkages.
  • Provide crisis management for clients, making linkages for interventions as appropriate.
  • Facilitate at least one clinical or life skills group in addition to morning group.
  • Maintain client and program records in accordance with applicable standards and regulations, grant requirements, etc.
  • Maintain a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature.
  • Participate in continuing education activities, remaining knowledgeable in area(s) of expertise.
  • Attend weekly case consultation meetings and monthly training, meeting regularly with supervisor to exchange pertinent information and receive supervision.
Requirements:
  • Bachelor's degree in social work, psychology, or related field, with a minimum of two years' case management experience.
  • Excellent communication and interpersonal skills, with proven ability to collaborate with community partners.
  • Understanding of the issues faced by homeless individuals and other marginalized populations in crisis.
  • Availability to work occasional evenings and weekends.
  • Experience working with mentally ill and dually diagnosed clients highly desirable.
  • An understanding of ideas and practices that promote diversity, equity, and inclusion, and a commitment to ongoing professional development in this area.
  • Bi-lingual Spanish language capabilities a plus.
