POSITION SUMMARY:
The Client Care Specialist serves as a vital member of the direct support team, assisting individuals in the Momentum Resource Solutions housing programs to achieve greater independence within the community.
PRIMARY RESPONSIBILITIES:
Resident Assistance
- May manage a caseload of clients.
- Delivers services to clients in alignment with program guidelines and organizational mission.
- Assesses and evaluates independent living capabilities, aiding clients in reaching personal objectives.
- Acts as an advocate for clients when interacting with other service providers.
- If managing a caseload, connects clients with community-based treatment and collaborates with other service providers to coordinate care.
- If managing a caseload, engages in person-centered and collaborative service planning with clients, ensuring that service plans are completed and signed in accordance with regulatory standards.
- Clarifies and explains organizational policies and procedures to clients.
- Guides clients towards achieving medication management independence through appropriate, individualized interventions.
- Conducts regular inspections of living units/apartments.
- Provides transportation support for clients to appointments as necessary.
- Assists clients with moving, carrying groceries, laundry, and other household tasks as required.

Knowledge and AttributesUnderstanding of the impact of mental health issues, substance use disorders, or trauma on individuals in the community.Experience working with individuals facing serious mental health challenges, substance use disorders, or trauma is essential.Familiarity with local and state health and human service systems and their navigation.Awareness of the complexities involved in stabilization and recovery processes.Commitment to supporting individuals with serious mental health conditions, substance use issues, a history of trauma, and/or homelessness.Compassion and respect for individuals in vulnerable situations.Willingness to approach work through an anti-racist perspective, engaging in ongoing learning to support a diverse client population.Ability to maintain composure in various situations, including those that may be stressful.Patience and empathy for individuals on their recovery journey.

EDUCATIONAL/TRAINING REQUIREMENTS:Associate's degree with two years of experience in a community mental health setting orHigh School diploma or equivalent with at least five (5) years of experience in a community mental health environment.
COMPUTER SKILLS REQUIRED:Proficiency in Microsoft Office suite (Excel, Word, PowerPoint, etc.).Willingness and ability to learn new systems and software applications.
