This position is essential for expanding the nursing center's clientele by optimizing occupancy rates and skilled mix from designated hospital accounts while exploring potential unassigned accounts.
Key Responsibilities:
* Make informed patient acceptance and placement decisions for Genesis, ensuring effective communication with referral sources in alignment with established performance metrics.
* Facilitate safe and efficient patient transfers across the healthcare continuum, acting as a liaison between hospitals, nursing centers, and patients or their families, with the aim of minimizing rehospitalizations and elevating customer satisfaction.

Qualifications:
Educational/Vocational Requirements:
1. RN License in the state of employment, with a willingness to obtain licensure in additional states as needed; BSN preferred.
2. Valid driver's license.
3. A minimum of seven years of recent clinical experience.
4. Availability 24/7 to meet hospital demands.
5. Experience in coaching clinical teams.
6. Required case management experience.
7. Preferred experience in business development.
8. Excellent written, verbal, and listening communication skills.
9. Prior experience in utilization review or discharge planning is preferred.
Certifications:
CCM, CRRN, or CIRS preferred.
