This role, under the guidance of the PACE Executive Director, is essential in ensuring the delivery of high-quality care and operational efficiency within the PACE program. The position demands expertise in assessing, planning, organizing, and evaluating participant care, while fostering a culture of continuous improvement and system thinking. The individual is expected to exemplify leadership qualities and support the professional growth and satisfaction of team members.

Key Responsibilities:

  • Maintain a comprehensive understanding of relevant PACE Federal, State, and local regulations, alongside the Compliance Accountability Program and Code of Ethics, ensuring ethical and professional conduct.
  • Oversee adherence to clinical standards of practice among PACE interdisciplinary team members, ensuring service delivery aligns with established benchmarks.
  • Lead the formulation, execution, and assessment of PACE service objectives and outcomes.
  • Evaluate and enhance the quality of care provided by the interdisciplinary team and external service providers.
  • Support the professional development of team members by identifying essential competencies and conducting performance assessments through collaborative evaluations.
  • Encourage the formation of strong, effective teams through adept communication, problem-solving, and financial acumen.
  • Conduct performance evaluations in accordance with UnityPoint Health standards, ensuring timely feedback and development opportunities.
  • Contribute to organizational and program planning as a member of the PACE Leadership team.
  • Monitor financial reports and identify opportunities for operational improvements.
  • Facilitate departmental meetings and conduct individual employee check-ins.
  • Implement Quality Initiatives as assigned.
  • Manage Human Resource functions for assigned staff, including scheduling and payroll approvals.
  • Ensure compliance with risk management and feedback resolution protocols.
  • Foster collaborative relationships with community health care providers.
  • Guarantee a safe and welcoming environment for participants and staff.
  • Participate in the on-call rotation as part of the Administrative team.
  • Perform additional duties as required.

Qualifications:

Education:

Bachelor's degree required or a combination of education and relevant experience.

Experience:

A minimum of three to five years of progressive management and leadership experience is essential.

Licenses/Certifications:

Valid driver's license for work-related travel and valid auto insurance are required. Must maintain current Mandatory Reporter status.

Knowledge, Skills, and Abilities:

Strong interpersonal skills to effectively engage with PACE participants, families, and interdisciplinary care providers. Excellent oral and written communication skills. Proven ability to lead within an interdisciplinary environment. Strong organizational capabilities and adaptability to changing priorities. Proficiency in Microsoft Office applications.
