Job Summary

UnityPoint Health is seeking a skilled Physical Therapist to join our PACE team. As a Physical Therapist, you will play a vital role in evaluating and treating patients requiring services associated with physical therapy.

The ideal candidate will have a strong background in physical therapy and excellent communication skills. You will work closely with the PACE interdisciplinary team to develop and implement treatment plans that meet the unique needs of each patient.

Responsibilities
  • Conduct thorough examinations and evaluations to determine patient diagnoses and develop effective treatment plans.
  • Utilize a range of physical therapy interventions to achieve optimal patient outcomes.
  • Communicate effectively with the PACE team, including physicians, nurses, and social workers, to ensure seamless care coordination.
  • Maintain accurate and detailed documentation of patient progress and treatment plans.
  • Supervise Physical Therapist Assistants as needed to ensure high-quality patient care.
Requirements
  • Graduate of an accredited physical therapy program.
  • Valid driver's license and auto insurance.
  • Licensed by the state of Iowa.
  • Ability to work effectively in an interdisciplinary team environment.
