Job Title: Private Duty Nurse LPN

Aveanna Healthcare is seeking a dedicated and compassionate Private Duty Nurse LPN to join our team. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, assisting them with transitioning from hospital to home and providing continuous care to ensure their well-being.

Responsibilities:
  • Implement physicians' prescribed plans of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in a timely and compliant manner
  • Teach and train family members on health promotion and disease management
  • Communicate effectively with physicians, clients, family members, and staff
Requirements:
  • Active, unencumbered LPN license in the state of practice
  • CPR certification from American Red Cross or American Heart Association
  • TB skin test (current within last 12 months)
  • 6 months prior hands-on nursing experience preferred
  • Reliable transportation
