Community Specialist Job Description

Key Responsibilities:

  • Develop positive relationships with individuals and their families, promoting self-determination and empowerment.
  • Assess individual needs and create personalized plans to achieve their goals and dreams.
  • Provide respite care and support to caregivers, ensuring they receive the help they need.
  • Connect individuals with community resources and services, enhancing their quality of life.
  • Collaborate with multidisciplinary teams to ensure comprehensive care and support.

Requirements:

  • High school diploma required; experience working with adults with mental health diagnoses and/or developmental disabilities preferred.
  • Valid North Carolina driver's license.
  • Ability to work flexible hours, including evenings and weekends.
