Role Responsibilities:The Rehabilitation Specialist is responsible for evaluating, planning, organizing, and implementing therapeutic programs aimed at enhancing mobility, alleviating discomfort, increasing strength, and addressing disabling conditions resulting from health issues or injuries.
As a crucial member of our therapy team, you will play a significant role in assisting residents in achieving their rehabilitation objectives and enhancing their overall quality of life.
Qualifications:- Education: Must be a graduate of an accredited therapy program.
- Experience: 2-3 years of relevant experience preferred; or an equivalent combination of education and experience.
- Licenses/Certifications: Must hold a current state license as a Physical Therapist and CPR certification.
- Restores patient functionality, alleviates pain, and prevents disabilities through the execution of medically prescribed physical therapy.
- Ensures high-quality care by assessing and interpreting evaluations and test outcomes.
- Creates individualized Treatment Plans for each patient based on initial and ongoing assessments, including both short- and long-term objectives, collaboration with other staff, and necessary involvement of patients and caregivers.
- Determines physical therapy treatment strategies in collaboration with physicians or via prescription.
- Implements Treatment Plans consistently, adjusting schedules and plans based on patient progress.
- Facilitates patient achievement of treatment goals through manual exercises, massages, and necessary equipment usage.
- Assesses mobility, strength, and current functional levels of patients, setting realistic and attainable improvement goals.
- Monitors and documents patient progress, modifying treatment plans as needed and exploring new treatment options when required.
- Evaluates the use and fit of prosthetic and orthotic devices, observing patient progress and recommending necessary adjustments.
- Documents patient care services in accordance with established medical and facility documentation protocols.
- Communicates patient progress to patients, caregivers, facility staff, and healthcare professionals involved in ongoing care.
- Develops post-treatment plans for patients, coordinating appropriate in-home care services.
