Job Summary

Providence is seeking a skilled and compassionate LPN to join our Home Health team. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes. This is a full-time opportunity to make a meaningful difference in the lives of our patients and their families.

Responsibilities
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Conduct thorough assessments of patients' physical and emotional needs, and provide education and support to patients and their families.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Current LPN licensure in the state of Washington.
  • 1+ year of experience in home health or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Basic computer skills and proficiency in electronic medical records.
