Speech Therapist Home Care Job Description

At MercyOne Home Health Care and Hospice Agency, we are seeking a skilled Speech Therapist to join our team. As a Speech Therapist, you will provide professional speech and audiology diagnostic and therapeutic services to our clients in the comfort of their own homes.

Key Responsibilities:
  • Assess clients at admission and at required time points, utilizing appropriate standardized documentation.
  • Develop and implement individualized plans of treatment with the client and physician, including attainable episode goals and agency services needed to improve and/or stabilize the client's medical condition and promote independence.
  • Make referrals for evaluation/care to other disciplines and services, and coordinate care with others to ensure effective and efficient care is provided.
  • Initiate and implement preventive, therapeutic, and rehabilitative procedures and techniques in accordance with the plan of treatment.
  • Assist and instruct clients and family members on methods of helping clients improve communication, swallowing, or other deficits.
Requirements:
  • Meets the educational and experience requirements for a Certificate of Clinical Competence in Speech Pathology granted by the American Speech and Hearing Association.
  • Registration or license to practice as a speech pathologist in the state of Iowa.
  • Must accept responsibility for maintaining clinical practice skills, learning, and adhering to agency policies and procedures on an ongoing basis.
  • Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with the agency's clients and their families.
