About the Role

Swedish Medical Center is seeking a skilled RN Case Manager to join our team. As a RN Case Manager, you will be responsible for coordinating services through an interdisciplinary process, providing a clinical and psychosocial approach to patient care.

Key Responsibilities

  • Assess patients to determine appropriateness of admission, continued hospitalization, and appropriate level of care.
  • Conduct discharge planning that begins at the time of admission and is reassessed throughout the course of hospitalization.
  • Monitor Quality and Risk Management issues and report to the QRM department as part of the CQI process.

Requirements

  • Current licensure in the State of Colorado as a Registered Nurse, or current active multistate nursing licensure.
  • BLS certification through AHA or ARC.
  • Graduate of an accredited school of nursing; BSN preferred.
  • 3-5 years recent clinical or case management experience in area of specialization.
  • Excellent human relations and oral/written communication skills.
  • Ability to assess information, apply critical thinking processes, and evaluate effectiveness of plan through continuous assessment of patient progress.
