Nurse Practitioner

Gifford Medical Center is seeking a skilled Nurse Practitioner to join our Primary Care Division. This position provides care to patients transitioning from an inpatient or ED stay back to their primary care clinician. The Nurse Practitioner will review, educate, and direct patients to ensure their understanding of their care plan. This role requires strong communication skills, accuracy, and compassion in a fast-paced setting.
This is a unique opportunity to work in a rural, financially stable, non-profit hospital with a progressive philosophy and supportive administrative team. - Onsite Parking and childcare
• Providing care to patients with recently discharged from a hospital stay or ED visit
• Educating patients and their families about their condition and what they should expect from the treatment plan
• Performing examinations and diagnostic tests to assess the patient's condition
• Communicating with their Primary Care Clinician to determine the best care plan for every patient
• Providing acute care to patients as needed
• Keeping records that track patient progress
• VT Licensure
• Ability to work with a high degree of accuracy and compassion in a fast-paced setting
