Overview:
The Transplant Nurse Coordinator (TNC) plays a crucial role in managing the comprehensive care plan for transplant patients of specific organ(s), starting from the initial referral as a potential recipient through to lifelong post-transplant follow-up.
The TNC operates under the general oversight of the Clinical Program Director, is accountable to the physician director of the transplant program, and exercises self-direction.
Key Responsibilities:
- Administrative Duties: Collaborates with the transplant program administrator in strategic planning for the transplant program through regular staff meetings and individual program discussions.
- Quality Improvement: Engages in the development, execution, and assessment of the transplant program's Continuous Quality Improvement (CQI) initiatives aimed at enhancing patient and graft survival outcomes. Reviews resource utilization with administrative and clinical teams to pinpoint areas for enhancement. Contributes to the creation and refinement of clinical pathways.
- Patient Care Coordination: Oversees and coordinates the clinical, psychological, and financial aspects of patient care. Manages pre-operative assessments and consultations to evaluate transplant eligibility. Maintains the program's waiting list, ensuring consistent follow-up on patient status during the waiting period. Facilitates the transplant process by coordinating between the patient and the transplant team. Monitors inpatient conditions, discharge requirements, and follow-up care in accordance with program standards.
- Patient Education: Informs patients and their families about the transplant process and what to anticipate at each stage. Responsible for developing or sourcing educational materials and delivering educational sessions to patients, staff, other healthcare professionals, and community organizations.
- Research Contributions: Gathers and maintains clinical data within the transplant office records and database necessary for research and reporting to external agencies.
Additional Information
Education Requirements:
- Bachelor's degree in Nursing (BSN) from an accredited nursing program.
- Licensed as a Registered Nurse in the state of North Carolina.
- Minimum of three (3) years of professional nursing experience, including at least two (2) years in a specialized area.
