As a vital member of the BCN PACE "Program of All-Inclusive Care for the Elderly," you will engage in delivering comprehensive healthcare services to elderly participants who can maintain their independence at home. This role allows you to build long-term relationships with participants, significantly enhancing their quality of life. Our dedicated PACE professionals collaborate within an interdisciplinary team to determine the best care strategies, connecting participants with essential medical, social, and support services, whether at home or in the PACE Center. Role Responsibilities:
The PACE LPN is responsible for delivering both direct and indirect nursing care to assigned participants in various settings, including the PACE Center, home, hospital, nursing home, or assisted living facilities. You will operate under the guidance of the PACE Center Director or Home Care Manager and the Registered Nurse.

Qualifications:
  • Graduate from an accredited nursing or vocational nurse program.
  • Current LPN licensure through the State Board of Nursing.
  • Minimum of one year of experience in geriatric care.
  • Food handling permit required, to be obtained upon hire.
  • Ability to operate equipment with visual and auditory components.
  • Capability to access various environments, including homes and healthcare facilities.
  • Strong communication skills, both verbal and written.
  • Ability to engage positively with participants, families, and external agencies.
  • Proficient in managing multiple tasks and handling emotional stress effectively.
  • Knowledge of primary medical care principles.
  • Demonstrated ability to provide care and communicate effectively with PACE participants.
  • Understanding of growth and development principles across the lifespan.
  • Collaborative mindset to work with PACE staff and volunteers.
  • Flexibility to adapt to changing participant needs and schedules.
  • Must possess a valid driver's license and have a reliable, insured vehicle.
  • Compliance with health, infectious disease, and vaccination policies is expected.
Salary Range:
$23.81-$29.36 per hour
