As a vital member of the Bluegrass Care Navigators PACE Program, you will contribute to a comprehensive healthcare initiative designed for elderly participants who can maintain their independence in their own homes.

Working within our PACE Center allows you to build long-term relationships with participants, significantly enhancing their quality of life.

Role Overview:

The PACE LPN delivers both direct and indirect nursing care to assigned participants across various settings, including the PACE Center, home, hospital, nursing home, or assisted living facilities.

This position operates under the guidance of the PACE Center Director or Home Care Manager and the Registered Nurse.


Qualifications:
  • Graduate of an accredited nursing or vocational nursing program.
  • Current LPN licensure through the State Board of Nursing.
  • Minimum of one year of experience working with elderly and frail populations.
  • Food handling permit required, to be obtained upon hire.
  • Ability to operate equipment with visual and auditory components.
  • Physical capability to access various environments (homes, hospitals, nursing homes, etc.).
  • Strong communication skills, both in-person and through digital means.
  • Ability to interact positively with participants, families, and other organizations.
  • Capability to manage multiple tasks and handle the emotional demands of the role.
  • Knowledge of primary medical care principles and practices.
  • Demonstrated skills in providing care and communicating effectively with PACE participants.
  • Understanding of growth and development principles across the lifespan.
  • Ability to work collaboratively with PACE staff and volunteers as part of the interdisciplinary team.
  • Flexibility to adapt to changing participant needs and schedules.

Must possess a valid driver's license and have access to a reliable vehicle that meets state and organizational insurance requirements.
