Job Summary

The Licensed Practical Nurse (LPN) plays a vital role in the Assisted Living Program, working under the direction of a qualified Registered Nurse (DON) to plan, implement, and evaluate nursing care. This position requires a strong understanding of the program's rules and regulations, as well as excellent communication and supervisory skills.

Responsibilities
  • Develop and maintain written objectives, standards, procedures, and organizational plans for delivering nursing services.
  • Provide supervision to Certified Medical Assistants (CMAs), Certified Home Health Aides (CHHAs), Certified Nursing Assistants (CNAs), and Housekeepers.
  • Oversee staff attendance, check phone messages, and return calls as needed.
  • Review new faxes, fax orders to MDs, pharmacies, and other relevant parties.
  • Read emails, communication books, and daily log books, addressing any concerns.
  • Check resident schedules for MD and other appointments, complete progress reports, and send them to MDs.
  • Review narcotic counts, ensure Nursing Aides document communications properly, and supervise their implementation of resident care plans.
  • Prepare, administer, and record medications according to policy.
  • Review new MARS, address changes, and review with RN prior to signing.
  • Order new medications, document appropriately, and ensure Nursing Aides complete monthly weights and vital signs.
  • Review monthly weights and vital signs, report deviations to RN and MD, and document accordingly.
  • Make weekly contact with residents, either in person or via telephone, and document unusual incidents, changes in condition, new orders, and medication changes.
  • Review labs, x-rays, and other relevant documents, fax copies to MDs, and review with RN.
  • Share responsibility with RN for developing and implementing care plans and health service plans, as well as coordinating all required healthcare services.
  • Assess, observe, and monitor residents' responses to treatment and nursing care, documenting information concisely, accurately, and completely.
  • Complete face sheets, UTF forms, and mini service plans upon admission and update as needed.
  • Keep emergency binders and immunization forms up to date.
  • Teach, supervise, and consult with other personnel, residents, and family members regarding nursing care needs and other related problems.
  • Ensure transportation arrangements are scheduled in advance for residents' medical appointments and send written reminders.
  • Respond to and participate in emergency situations, call 911 if needed, and notify RN, residents' MD, and Administrator.
  • Ensure residents have initial Physician Certification (PA-4) forms, History and Physicals, and listed diagnoses upon admission, and document annual physicals.
  • Update and maintain resident emergency packets, communicate with RN and Administrator regarding resident and staff issues, and attend monthly team meetings.
  • Review charts for accuracy, completeness, and thinning as needed, and ensure discharge plans are in place.
  • Encourage independence in activities of daily living, abide by established guidelines, and maintain confidentiality of verbal and written information.
  • Have a full understanding of fire, emergency, and safety procedures, and assist in the orientation of new personnel.
