Job Summary:

Alaris Health at Hamilton Park, a 260-bed skilled nursing facility in Jersey City, New Jersey, seeks a licensed practical nurse (LPN) to provide high-quality resident care, treatment, and services. The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Key Responsibilities:

Provide direct resident care, including administering medications, wound care, and vital sign monitoring
Develop and implement individualized care plans, working collaboratively with the interdisciplinary team
Maintain accurate and detailed records of resident care and treatment
Participate in resident assessments, care planning, and goal setting
Collaborate with nursing staff to ensure seamless care transitions and continuity
Maintain a safe and clean environment, adhering to infection control and safety protocols

Requirements:

Graduate of an approved school for practical or vocational nursing
Current licensure by the New Jersey State Board of Nursing
Rehabilitative or geriatric nursing experience preferred
Possess manual dexterity, visual and aural acuity, and excellent communication skills
Ability to work effectively in a team environment, with a focus on resident-centered care
