Personal Care Specialist

We are seeking a skilled and compassionate Personal Care Specialist to provide high-quality nursing care to medically fragile patients in the comfort of their own homes.

This is a full-time opportunity for a Registered Nurse (RN) or Licensed Practical Nurse (LPN) to work as part of our team providing personalized care to patients in Goode, VA. As a Personal Care Specialist, you will be responsible for delivering skilled nursing services to patients with complex medical needs, ensuring their safety and well-being at all times.

• Provide high-quality nursing care to patients in their homes, including administering medications, wound care, and other treatments as prescribed by physicians.
• Conduct thorough assessments of patients' medical conditions, identifying potential risks and developing individualized care plans to mitigate these risks.
• Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care coordination.
• Maintain accurate and detailed records of patient care, including progress notes, medication lists, and treatment plans.

• Current licensure as an RN or LPN in the state of Virginia.
• At least 1 year of experience in home health care or a related field.
• Ability to lift up to 50 pounds, stand for long periods, and move quickly and safely in response to patient emergencies.
• Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
