About the Role

As a Clinical Nurse Practitioner at Matrix Medical Network, you will play a critical role in improving quality of care and closing gaps in patient health. Our team conducts comprehensive health assessments in various settings, including homes, skilled facilities, and telehealth visits.

You will work collaboratively with physicians, case managers, social workers, family members, and other healthcare professionals to provide holistic care and support to patients. Your expertise as a Nurse Practitioner will be essential in diagnosing and treating conditions, educating patients on healthy practices, and making referrals when necessary.

Key Responsibilities
  • Conduct thorough health assessments, reviewing medical history, medication adherence, social environment, and other factors that impact patient well-being.
  • Develop and implement personalized care plans, working closely with patients and their families to ensure optimal health outcomes.
  • Maintain accurate and detailed records of patient interactions, treatments, and follow-up actions.
  • Collaborate with primary care physicians and other healthcare professionals to coordinate patient care and ensure seamless transitions.
  • Participate in ongoing professional development and training to stay current with best practices and advancements in healthcare.
Requirements and Qualifications

To be successful in this role, you must possess:

  • A Master's Degree in Nursing or a related field, or equivalent experience and licensure.
  • Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain it.
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • A minimum of one year of experience as a Nurse Practitioner.
  • Strong computer skills and familiarity with electronic health record systems.
  • Excellent verbal and written communication skills, with the ability to interact effectively with patients, colleagues, and other stakeholders.

  • A base salary of $90.00 per completed assessment.
  • Flexible scheduling, with opportunities for part-time or PRN work.
  • Mileage reimbursement for travel-related expenses.
  • No show payment for booked assessments.
