About the Role:We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in the Southwest Twin Cities Metro area. As an LPN, you will have the opportunity to work independently and collaboratively with our interdisciplinary team to provide high-quality care to patients in their homes.
Responsibilities:- Participate in ongoing interdisciplinary assessment of the patient.
- Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
- Provide services that are ordered by the physician as indicated in the plan of care.
- Recognize and report changes in the patient's condition to the RN.
- Counsel and educate the patient and family in meeting healthcare related needs.
- Complete timely and accurate clinical notes including addressing patient's progress.
- Current license as an Licensed Practical Nurse (LPN) in the State of MN.
- License is active and in good standing.
- 2 years' experience as an LPN.
- Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance.
- CPR Certification.
- Competitive pay rate of $35.88/hour.
- $6,000 hiring bonus.

This position will work in the field helping patients in their home setting. Our office is in Roseville, MN, and the general territory for this position would be the Southwest Twin Cities Metro area.
