Pharmacist Clinician

Job Summary:
The Pharmacist Clinician is a key member of the ambulatory care team, working collaboratively with patients and healthcare providers to optimize medication therapy and improve health outcomes.
Responsibilities:
- Conduct comprehensive medication reviews to ensure safe and effective medication use
- Develop and implement individualized medication therapy plans
- Collaborate with healthcare providers to coordinate care and address patient needs
- Provide education and support to patients and caregivers on medication management and self-care
- Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements:
- Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE)
- Completion of a PGY1 or PGY2 ambulatory care residency or at least 2 years of experience in ambulatory clinical pharmacy
- Board certification in a relevant specialty (BCACP, BCPS, or BCGP)
- Active license to practice pharmacy in MN and WI
- BLS certification
