Job Summary

We are seeking a highly skilled Pharmacist Clinician to join our team at Essentia Health. As a key member of our ambulatory care team, you will work collaboratively with patients and healthcare providers to optimize medication therapy and improve patient outcomes.

Key Responsibilities
  • Conduct comprehensive medication reviews to ensure patient safety and effectiveness
  • Develop and implement individualized medication plans to improve patient health outcomes
  • Collaborate with healthcare providers to ensure seamless care transitions and coordination
  • Provide education and support to patients and caregivers on medication management and self-care
Requirements
  • Graduation from an ACPE-accredited pharmacy program
  • Completion of a PGY1 or PGY2 ambulatory care residency or 2+ years of experience in ambulatory clinical pharmacy
  • Board certification in a relevant specialty (BCACP, BCPS, or BCGP)
  • Active licensure to practice pharmacy in MN and WI
