Job Title: LPN Licensed Practical Nurse Homecare

Maxim Healthcare Services is seeking a skilled Licensed Practical Nurse (LPN) to join our team in Indianapolis, IN.

About the Role:

We are looking for a dedicated and compassionate LPN to provide high-quality patient care in a homecare setting. As an LPN, you will be responsible for assessing, planning, implementing, and evaluating patient care, ensuring the best possible outcomes for our patients.

Responsibilities:
  • Assess signs and symptoms indicating physiologic and psychosocial changes in patients' conditions.
  • Collect, analyze, and interpret data from healthcare members and document actual and/or potential nursing diagnoses.
  • Develop and implement individualized patient care plans, prioritizing patient needs and goals.
  • Perform interventions according to identified priorities, plan of care, and hospital policies and patient care outcome standards.
  • Re-evaluate patient care plans and make adjustments as needed, ensuring effective and efficient care.
Requirements:
  • Current Licensed Practical Nurse (LPN) License in the state of Virginia or compact.
  • Current Health Certificate (per facility).
  • Current PPD or Chest X-Ray.
  • Current BLS card.
  • One year prior LPN experience preferred.
