Job Summary

We are seeking a skilled Licensed Practical Nurse (LPN) to join our hospice care team. As a Hospice LPN, you will play a vital role in providing compassionate and personalized care to patients in their homes. Responsibilities
  • Deliver high-quality patient care in accordance with physician-ordered plans of care
  • Assist Case Managers in ongoing observation and care of assigned patients
  • Communicate effectively with patients, families, and healthcare team members
  • Document patient observations, interventions, and evaluations in a timely and accurate manner
  • Participate in on-call, holiday, and weekend rotations as assigned
Requirements
  • Current licensure to practice as a Licensed Practical Nurse in the State of Oklahoma
  • Previous hospice nursing experience preferred
  • Certification in Hospice and Palliative Nursing is preferred but not required
  • Excellent verbal and written communication skills
  • Ability to maintain concentration during frequent interruptions
