Responsibilities- Deliver high-quality patient care in accordance with physician-ordered plans of care
- Assist Case Managers in ongoing observation and care of assigned patients
- Communicate effectively with patients, families, and healthcare team members
- Document patient observations, interventions, and evaluations in a timely and accurate manner
- Participate in on-call, holiday, and weekend rotations as assigned
- Current licensure to practice as a Licensed Practical Nurse in the State of Oklahoma
- Previous hospice nursing experience preferred
- Certification in Hospice and Palliative Nursing is preferred but not required
- Excellent verbal and written communication skills
- Ability to maintain concentration during frequent interruptions
