Job Summary

As an Individualized Service Coordinator, you will work collaboratively with our team to provide individualized support to clients, focusing on skill development and community integration.

Responsibilities
  • Develop and maintain relationships with clients and their families
  • Collaborate with other professionals to create personalized support plans
  • Provide ongoing support and coaching to clients to achieve their goals
Requirements

To be successful in this role, you will need:

  • A minimum of a High School Diploma or General Equivalency Diploma (GED)
  • Compassion and a willingness to care for the well-being of others
  • Excellent communication and interpersonal skills
  • A valid Driver's License and existing insurance
