Job Title: Case Manager RN

CVS Health is seeking a skilled and compassionate Case Manager RN to join our team. As a Case Manager RN, you will play a vital role in facilitating the delivery of benefits and healthcare information to our members, promoting wellness activities, and developing strategies to ensure successful and timely return to work.

Key Responsibilities:
  • Facilitate the delivery of benefits and healthcare information to members
  • Develop and implement health strategies, policies, and programs to promote member wellness and successful return to work
  • Collaborate with healthcare providers and other stakeholders to ensure seamless care coordination
  • Conduct thorough assessments and develop personalized care plans to meet the unique needs of each member
Requirements:
  • Active and unrestricted licensure in CA and NV
  • Minimum 3+ years of clinical experience, preferably in diabetes, CHF, CKD, post-acute care, hospice, palliative care, or cardiac care
  • Minimum 2+ years of experience in case management, discharge planning, and/or home health care coordination
  • Excellent analytical and problem-solving skills
  • Effective communication, organizational, and interpersonal skills
  • Ability to work independently and efficiently
Preferred Qualifications:
  • Proficiency in standard corporate software applications, including MS Word, Excel, Outlook, and PowerPoint
  • Ability to navigate multiple systems and keyboard efficiently
  • Willingness to obtain multi-state RN licenses if needed
Education:
  • Associates Degree or Nursing Diploma Required
  • Bachelors Degree Preferred
Pay Range:

The typical pay range for this role is $60,522.80 - $136,600.00. This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors, including experience, education, geography, and other relevant factors.

This position is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range listed above.
