Job Summary

We are seeking a highly skilled MDS Coordinator to join our team at St. Luke's Hospital Transitional Care Center.

Key Responsibilities
  • Implement facility policies and procedures, adhering to established standards of nursing practice.
  • Manage the MDS schedule to meet Medicare and OBRA requirements, ensuring timely and accurate completion of assessments.
  • Conduct thorough resident interviews, physical assessments, and medical record reviews to inform comprehensive care plans.
  • Collaborate with interdisciplinary teams to facilitate effective care transitions and communicate discharge plans.
  • Audit charts for completion of required documentation and gather information for Medicare meetings.
  • Develop and update resident care plans, ensuring timely updates and accurate information.
  • Conduct bedside assessments, including Braden, Fall Risk, and Active TB assessments, on admission.
  • Review admission charts for completion and ensure accurate documentation.
Requirements
  • Proven experience as a Registered Nurse (RN) or similar role.
  • Ability to work in a multidisciplinary team and undertake diverse care tasks.
  • Familiarity with health, safety, and sanitation standards and procedures.
  • Understanding of confidentiality obligations and nursing best practices.
  • Excellent knowledge of medical and hospital terminology, as well as communication and interpersonal skills.
  • Compassionate and able to handle stress, with a successful completion of an RN license program.
